Certificate of insurance requirements for renting from Slaymaker Rentals.

Before you send your certificate
Please make sure the COI has the requirements below. If requirements are not correct, the document will not be accepted as a valid COI for Slaymaker Rentals.
Coverage requirements
- General Liability: $2,000,000
- The COI must name Slaymaker Rentals as Additional Insured.
- Slaymaker Rentals, 150 Penn Street, Washington Boro, PA 17582 must be in the certificate holder box.
- Auto Liability: $1,000,000
- Workers Comp: $500,000
- Must have an Umbrella Policy of $1,000,000.
- Leased and Rented Equipment Coverage in the amount of the value of the item rented.
